Application Criteria
Applicants must meet the following criteria to be considered for High Physical Support SDA:
NDIS Eligibility: Must be an active participant in the National Disability Insurance Scheme (NDIS).
SDA Funding: Have High Physical Support funding in NDIS plan (preferred: onsite overnight assistance-OOA)
Extreme Functional Impairment or Very High Support Needs: Applicants must demonstrate significant physical impairment that severely limits their ability to perform daily activities, even with assistive technology or home modifications.
Functional Assessment: A comprehensive assessment by an allied health professional is required to confirm the level of support needed.
Housing Goals in NDIS Plan: Applicants should have clearly defined housing goals in their NDIS plan that reflect the need for long-term, accessible, and supportive living arrangements.
Reasonable and Necessary Test: The NDIA must determine that High Physical Support SDA is a reasonable and necessary support to help the participant live more independently and safely.
