Heart In Hand Support Services (HIH) wishes to offer NDIS Plan Managed or Self-Funded individuals to reside in a beautiful private residence in a quiet beach side community, in Wurtulla Caloundra.

About us:

HIH specialise in tailored in-home support for people with neurological conditions (including dementia, MND, MS and other) and is dedicated to empowering people living with their disability to live their best life with positive engagement, with family members, support coordinators, plan managers and relevant allied health providers.

With compassion and understanding at the core of our approach we aspire to create a community of support where every person feels valued, respected, and cared for.

What we offer:

We offer a residential accommodation environment that supports individuals with high care need, who wish to remain in a homely environment with the tailored supports.

Residence includes:

- Two staff rooms (currently one in used)

- Two Client Bedrooms (currently one in used)

- 2 x Living Area with Television (currently one in used)

- Built in 24-hour security cameras.

- Airconditioning throughout

- Pool

- Single level with ramps access where needed

- Private and quiet court with limited traffic

- Access to beach, park, canal and walkways

- Supported 24 hours

- Veggie Garden

- Short term and Respite options available.

We have a team of multidisciplinary colleagues who include but not limited to nurses, AIN, personal trainers and experienced support staff trained in mental health and neurological backgrounds. We tailor to the individuals needs with a homely environment.
