Application Criteria
At Cosy Home Care, onsite shared supports within our Improved Liveability SDA apartments are delivered through a person-centred approach that promotes independence, choice, and quality of life. Support arrangements are established following a comprehensive assessment of each participant's NDIS goals, SDA eligibility, functional capacity, support requirements, preferences, and funding. While participants enjoy the privacy and independence of their own modern apartment, onsite shared supports are coordinated to ensure assistance is available when required while respecting each person's routines, privacy, and individual lifestyle. Support arrangements are regularly reviewed in partnership with participants, families or nominees, Support Coordinators, and allied health professionals to ensure supports remain responsive and aligned with changing needs. Reviews consider participant feedback, progress towards individual goals, health and wellbeing, risk management, and opportunities to build independence. Where changes are identified, support arrangements are adjusted to ensure participants continue to receive safe, effective, and person-centred support while enjoying the benefits of independent apartment living within a secure and supportive SDA community.
